You can expect 20/20 vision in 6-7 days. Vision steadily improves over 3 months to often better than 20/20.

Yes, true. LASIK takes 4 hours for the vision to recover. PRK or LASEK takes 4-7 days.

Thanks Olivia, yes you should be able to drive about 5 days after the PRK. Say we do the consult on Monday, op on Tuesday then back on Friday to have the contact lens removed. Vision usually pretty good by Saturday or Sunday. You should be able to fly back Sunday. I will be happy to do the procedure for you.

Dear David,

Thank you for being so straight with me. What a nightmare! I'm so sick of my glasses and I've never tolerated contact lenses well. I'll just have to plan very carefully and pick a quiet period in my husband's work schedule. If such a thing exists.

Would you expect a patient to be able to drive within a week of the operation - assuming things go smoothly? Or do I need to win the lottery as well and hire a driver for the duration? And would you do the surgery for me? How many days would I need to be in the UK for?

Dear Olivia
Your corneas are very thin and you should NOT have LASIK. Of course, you will be able to find doctors to operate on you with LASIK, but you should not do that. Certainly PRK/LASEK (they are basically exactly the same) can be painful on the second and third day, but anaesthetic drops will control most of that and do not delay healing as earlier thought. Your vision should be pretty good on the fourth day after treatment (contact protective lenses come out on the third day), but can take 6-7 days in some patients. Logistically this will be hard from your description at home, but PRK is the only way you should go. Wait until you have the situation where you can take several days to get this done and don't need to drive. It may not feel fair bit it is just how it is, I am afraid. Don't have LASIK.
Regards, Dave

Dear David, I wrote a while ago to ask whether Z Lasik gave greater scope to those of us with thin corneas for having Lasik as opposed to Lasek or PRK. At the time, I didn't know what my actual measurement was - I have since had my notes from my previous check and they read L 465, R 473. However, reading recent posts on your blog I see that you do not recommend any 'flap cutting' to anyone with a corneal thickness measurement of under 500.

I suppose what I'm asking, is if that is completely set in stone? Does Z Lasik not allow you to consider previously borderline cases? Am I realistically looking at Lasek? Does it hurt as much as everyone says it does?? And is the recovery as long? Realistically, I live in Switzerland at the moment, have 3 small children, a husband who travels all the time and I have to drive everywhere - making Lasek a logistic nightmare and possibly a complete no-no for the forseeable future!

Am I a hopeless case?

Regards, Olivia

Hi Thomas
Many surgeons, myself included, don't cut flaps in corneas less than 500 microns. So I would offer PRK and use mitomycin C to prevent haze.
Thanks

Hi Dave,
I have myopia of -6.5 and -2 for astigma. I have a corneal thickness of 490 micron. I went for 2 different consultations. One surgeon recommend that i should do PRK because he said that it is safer than iLasik (intralase) due to zero flap create. The other doctor recommended me to go with Intralase because he said that it is possible. I know that both procedures are safe. But i am just a bit concern in the long term. Also, what is the down side of having intralase over PRK or the other way around?

Thanks,

Dear John
Should be no signs to an optometrist that you have had PRK. You can have traces of haze in the first year but they will fade. We dont see the major haze from very high treatments now due to use of MMC to block haze in these cases.
Regards, Dave

Does PRK surgery leave a lasting scar? In other words, 5 years after the surgery, would an optometrist be able to examine my eyes and know that I had PRK surgery? Or will it fix the eye to make it appear like I was born with 20/20 vision?

Thanks,
John

Hi Irfan
Thank you for letting me know how great your vision is still, one year on from your Z-LASIK! Yes, of course happy to see you here for your 1 year check up.
Yes, those methods you mentioned are still essentially PRK. Ziemer Z-LASIK together with the WaveLight laser is still our recommendation for the best technology approach for LASIK. The Ziemer has upgrades through software just to make operation easier for the use, and also for complex non-LASIK procedures, e.g. ring inserts for keratoconus (an eye disease) though the core femto delivery is the same and still in my opinion the best available. I will ask one of my team to contact you to arrange.
With best regards
Dave
